Cardiovascular disease (CVD) is the leading cause of death and disability in the United States for men and women. This is the consequence of a population that continues to grow in obesity and inactivity. And the risks of CVD only increase when high cholesterol and hypertension are present.

Prevention: Your Number One Priority

As opposed to waiting for your heart catheterization or emergency triple bypass, instead focus on prevention. Instead of having your CVD  diagnosed by screening tests, such as an ECG, stress test, and cardiac catheterization procedures think about how you can prevent that from every happening.

Where to begin

First, it is imperative that you reduce your risk through lifestyle modification. This can be in the form of quitting smoking and adopting a healthy diet. Second, the emphasis is primarily on decreasing cholesterol and managing high blood pressure. Although all of these strategies are important, I would like to examine the role estrogen plays in the prevention of cardiovascular disease.

The critical role of estrogen

Foam cells are directly associated with atherosclerosis, which is fatty accumulation within the arterial walls. This Atherosclerosis causes an increased risk of heart attack and stroke. Simply put, Estrogen helps to decrease foam cell formation and the development of atherosclerosis. Estrogen has been shown to increase high-density lipids (HDL), which is the housecleaner for the arterial wall. The HDL removes the harmful cholesterol, and is considered the most important part of cardio-protection.

Additionally, estrogens have been shown to reduce the chemical reactions in the body that contribute to the formation of low-density lipids (LDL). The LDL cholesterol is referred to as the “bad” cholesterol, because too much is considered unhealthy. The protective effect of hormone replacement therapy may extend beyond its beneficial actions on lipids.
The benefit of estradiol is the direct effect on lowering Fibrinogen, which is closely tied to inflammation. As Fibrinogen levels increase so does the risk of stroke. Estrogen is the only medication that has been shown to lower Fibrinogen. An article published in 2009 in The American Journal of Medicine concluded that hormone replacement therapy certainly decreased mortality in younger postmenopausal women. The randomized trial and observational studies indicated that using hormone replacement in younger menopausal women decreased the risk of deaths coming from coronary heart disease, bone fracture, and colon cancer.

Understanding the different types of estrogen

Let me be clear that when I refer to estrogen in this article. I am describing estradiol, which is the bio-identical equivalent to the estrogen produced naturally by the body. This should not be confused with synthetic versions, such as Premarin. It has been clearly shown that using synthetic estrogens increases the risk of breast cancer, stroke, and pulmonary embolism. It is important that the risks and dangers associated with synthetic versions of estrogen not be extrapolated to include estradiol.
